The United Keetoowah Band of Cherokee Indians Tribal Council has declared a one-time, COVID-19 relief payment will be distributed to all enrolled, exclusive Keetoowah tribal members from the $24 million give to us through the CARES Act. Below are all the forms and steps required to complete the step by step guide to receiving funds.

You must determine if you are eligible, make sure all your information is updated and do so if not, fill out the Composite Form and W-9 and note if you are picking up in person or by mail. This will determine your disbursement date.

Step 1: Am I eligible?

Every exclusive Keetoowah 18-years-old and over (by May 16, 2020) will receive $1,500, while exclusive Keetoowah children 17-years-old and younger (by May 16, 2020) will receive $500 each.

You MUST have a Keetoowah membership card and roll number issued by our Enrollment Office to be eligible. You MUST have been an exclusive member at the time legislation was passed on May 16, 2020. 

 

If you are unsure if you are an exclusive Keetoowah or suspect you may be dual enrolled, you can contact our Enrollment Office on Wednesdays or Thursdays from 8:30 a.m. to 4:30 p.m. at 918-871-2797, or send an email inquiry to astopp@ukb-nsn.gov or jfourkiller@ukb-nsn.gov, to check your status.

 

What does "exclusive Keetoowah" mean? Keetoowah members that are classified as "exclusive" are those that hold membership with the United Keetoowah Band of Cherokee Indians in Oklahoma ONLY. These members do not hold a tribal membership with any other tribe and their names will only be found on our enrollment list. Some members can be classified as "dual enrolled," meaning that our Enrollment Office has checked the enrollment lists of other tribes and found the member in question to be an enrollee on both.

 

While not illegal, our Enrollment Ordinance does not permit dual enrollment in terms of providing services to members. This is in part to prevent "double-dipping" of federal funding given to our tribe and that of others. Most commonly, dual enrolled members are affiliated with the other two federally recognized Cherokee tribes - the Eastern Band of Cherokee Indians and the Cherokee Nation of Oklahoma.

At this time, the UKB Tribal Council will NOT be issuing checks to Keetoowahs who are dual enrolled. If you are dual enrolled and not eligible, you will receive a letter from Enrollment.

Step 2: I'm eligible. Is my information updated?

The United Keetoowah Band of Cherokee Indians Enrollment Office MUST have your information current and correct for mailing and check purposes.

If you are an exclusive Keetoowah and you know your name and address is current, there is NO NEED to contact us. If you receive an update information form in the mail but you have done so, please disregard. If your info is current, proceed to Step 3. If you need your information updated, follow the instructions below.

If you need to check your information is correct, please contact Enrollment via email at astopp@ukb-nsn.gov or jfourkiller@ukb-nsn.gov, or by calling 918-871-2797 on Wednesdays or Thursdays from 8:30 a.m. to 4:30 p.m.

If you need your information UPDATED, you MUST do so by filling out an Update Information form above. This form will need to be completed and signed by you and returned to our office using one of the methods below.

To return this form, please do one of the following:

1. Drop it off at the front of our Federal Programs Building in Tahlequah in our secure lockbox OR

2. Return by fax to our Enrollment Office at 918-414-4000 OR

3. Scan completed form and attach via email to astopp@ukb-nsn.gov or jfourkiller@ukb-nsn.gov OR

4. Return by mail to P.O. Box 746 Tahlequah, OK 74464 ATTN: Enrollment  OR

5. Deliver it to your district representative. Contact Information can be found by clicking HERE.

If you are unable to print from the provided download, you will need to pick up this form in person at our Federal Programs Building on our business days or request this form be mailed to you by contacting Enrollment at 918-871-2797 on Wednesdays or Thursdays from 8:30 a.m. to 4:30 p.m. You can also request this form be mailed to you by sending an email request to astopp@ukb-nsn.gov or jfourkiller@ukb-nsn.gov or by contacting your district representative.

 

Step 3: Complete Composite Form & W-9

The United Keetoowah Band of Cherokee Indians will require all exclusive members receiving a COVID-19 relief payment to fill out a Composite Form and a W-9 (downloadable above). 

 

You must note on the Composite Form if you will be picking up your check in person or need it mailed to you (more information on pickup in step 4).

 These forms need to be completed and signed by you and returned to our office using one of the methods below. If you have a child that is eligible, but they are over the age of 18, they will need to complete this paperwork ON THEIR OWN. Parents/guardians cannot legally fill out this information for them if they are 18 or older.

At this time we are NOT sending confirmation emails for documents received. If there is an issue, we will contact you. If you do not hear from us, please assume your information was received and will be ready for disbursement.

The date you turn in these forms will determine when you receive your funds! For disbursement dates, see Step 4. Please note, our Attorney General is not clear on the tax issues this presents yet. It will be up to each individual tribal member to decide whether to accept this payment given that there may be "tax consequences." You may wish to consult with your tax official should you be concerned.

Step 4: Disbursement/Mailing Dates

The date you return your composite form and W-9 will determine which date you are eligible to receive your payment in person or have it mailed to you. All checks picked up in person will require a signature. If you are having your funds mailed, the W-9 form will serve as your signature. 

At this time, Enrollment has not communicated a set timetable for payments that must be sent by mail. Please reach out to Enrollment if you have questions.

FORMS RETURNED JUNE 5TH - JUNE 11TH

If you returned your form between these dates, your relief payment will be ready for pickup on FRIDAY, JUNE 19 FROM 8:30 A.M. TO 4:30 P.M. Pickup will take place at the UKB Peace Pavilion at the very back of the loop on our grounds where we hold Celebration each year. We will have signs directing traffic. We ask that if you pick up your check in person, please be kind to everyone by practicing safe social distancing of 6-feet-apart and wearing a mask. Tables will be set up for members to find their checks by the first letter of their last name (ex. Table B for "Bunch"). Should there be inclement weather, pickup will be held at the Wellness Center and we will update our social. If you have arranged to pick up your check in person, but are now sick or showing symptoms, please contact our Enrollment Office at 918-871-2797 to make mailing arrangements instead.

FORMS RETURNED JUNE 12TH - JUNE 18TH

If you returned your form between these dates, your relief payment will be ready for pickup on FRIDAY, JUNE 26 FROM 8:30 A.M. TO 4:30 P.M. Pickup directions are the same as June 19th. Again, we ask you to practice safe social distancing and wear a mask due to the threat of COVID-19.

FORMS RETURNED JUNE 19TH - JUNE 24TH

If you returned your form between these dates, your relief payment will be ready for pickup on THURSDAY, JULY 2 FROM 8:30 A.M. TO 4:30 P.M. Pickup will be at the UKB Wellness Center, just past the Jim Proctor Elder and Nutrition Center on our loop. We will have signs directing traffic. Again, please follow safe social distancing guidelines and wear a mask please.

IF YOU MISS THESE DATES, the last opportunity for mail or pickup of your check will be JULY 10.
