The United Keetoowah Band will begin taking applications for its winter 2019 Elder Assistance Program on Tuesday, Nov. 12.

 

Assistance will be given in the form of $75 for exclusive Keetoowah elders 55 and older who have an updated enrollment form and a completed Elder Assistance application.

 

Applications can be obtained by visiting any of our main or satellite tribal offices, as well as by contacting your district representative or downloading the form at the top of this page. A W-9 is also needed as part of the application requirements and can be downloaded above.

 

Completed applications must be submitted in person or by mail to UKB Tribal Enrollment at 18263 W. Keetoowah Circle in Tahlequah. Faxed applications can also be accepted by sending it to 918-414-4004.

 

Elders have from Nov. 12 to Nov. 27 to complete the application for first disbursement, with the exception of weekends and tribal holidays. First disbursement checks will be mailed Dec. 6.

 

Second disbursement will also be available for elders who miss the Nov. 27 deadline and take place from Dec. 2 to Dec. 18, with the exception of weekends and tribal holidays. No applications will be accepted after Dec. 18. Second disbursement checks will be mailed Dec. 20.
